2010 Entry Forms
|
We are pleased to announce the following service to our competitors online. You can download competition Registration Forms (below) as well as Ticket Form. You will need "Adobe Acrobat Reader" to view and print these forms. Please read Event Rules, Policies, and Syllabus carefully before submitting entries and packages.
Mail all entries and packages with payment in full by money order or cashiers check in US currency made payable to "SAM SODANO" to:

Personal and studio checks will be returned. Payment must accompany all completed forms for entries no later then October 7, 2010. Entries received after that date, if accepted, will be subject to a $50.00 per person late fee. CREDIT CARDS NOT ACCEPTED
Over the phone, faxed or e-mail entries will not be accepted. Late entries will only be accepted if the timetable permits and does not create extra rounds. All late, or changed, or substituted entries will not be counted towards Top Teacher, Top Studio awards. Late entries received after OCTOBER 7TH could be returned. You will receive confirmation of your entries and packages in the mail. Please check very carefully that no mistakes have been made.
Cancellation/Refund Policy:
Cancellation must be in writing and made prior to October 30, 2010 to receive a refund 30 days after the event. Cancellation made after October 30, 2010 will NOT receive a refund. Canceled entries taken at the event cannot be substituted by another entry and no refund
will be made on such a cancellation. There are no refunds or exchanges on general admission tickets

Junior 1 Dance free-style entries. American or International $20. per dance
Junior American 3 Dance Smooth or Rhythm 30. per entry
Junior International 3 Dance Standard or Latin 30. per entry
Junior International 5 Dance Standard or Latin 35. per entry
Adult 1 Dance free-style entries. American or International 30. per dance

Solo Exhibitions (limit 2 per person) 60. per dance

Novice 2 dance - Bronze, Silver or Gold 30. per entry
Pre-Championship 3 Dance International
Standard or Latin - under or over 45 30. per entry

International 5 Dance Open or under 21, Standard or Latin 35. per entry

Theater Arts 90. per entry

Competition Attire as follows:
Tuesday will be casual.
Wednesday will be casual.
Thursday will be casual.
Thursday evening DanceSport Series Regional colors.
Region 1 - Orange
Region 2 - Blue
Region 3 - Purple
Region 4 - Yellow
Region 5 - Green
Region 6 - Red
Region 7 - Gold
Friday evening will be semi-formal.
Saturday evening will be formal.
Sunday will be casual
