Junior 1 Dance free-style entries. American or International $20. per dance
Junior American 3 Dance Smooth or Rhythm 30. per entry
Junior International 3 Dance Standard or Latin 30. per entry
Junior International 5 Dance Standard or Latin 40. per entry
Adult 1 Dance free-style entries. American or International 30. per dance
*(if not on package) 35. per dance
Solo Exhibitions (limit 2 per person) 70. per dance

Novice 2 dance - Bronze, Silver or Gold 40. per entry
Pre-Championship 3 Dance International
Standard or Latin - under or over 45 40. per entry
Over 35 International Standard or Latin 50. per entry
Senior International Standard or Latin (over 50) 50. per entry
International 5 Dance Open or under 21, Standard or Latin 50. per entry

PROFESSIONAL
Rising Star $75. per entry
Theater Arts 100. per entry
World Smooth or Rhythm 100. per entry
Open Standard or Latin 100. per entry
Professional entry fee includes admission to ballroom only when you are competing in professional event. Professional entry fees will be waived if the Professional couple has twenty or more Pro/Am entries for each professional division entered.
Please send payment in full by money order or cashiers check in US currency made payable to "SAM SODANO”. Personal and studio check will be sent back. Payment must accompany all completed forms no later then October 7, 2011. Forms received after that date, if accepted, will be subject to a $50.00 per person late fee.
